Is it still confusing if you think of it as a dream? If you think of it as a dream, Dean is presumably asleep in his bed and experiencing all this as a world in his dreams. In "Dreamwalking" it was revealed that Sam has the ability to manipulate dreams, so ... might this be one such dream?
