As for the naming issue, you'll have to tell me/think about context. The nicer Sams in his dreams don't seem to mind being called Sammy, but I don't know if he's ever called the evil!Sam out in the real world "Sammy" before. As for "Sam," I think some of it is just about the tone in which he says it. It seems to be a form of address that Dean uses when he's trying to get through to his brother/lover/the-old-Sam, and whether or not evil!Sam recognizes that distinction, it may be that he doesn't get upset because there's still some of the-old-Sam left in there.
Too ... as harsh as evil!Sam can be sometimes, he can also be tender and reasonable, so ... I'm not sure if he's actually as much a stickler for every single rule all the time. Some seem to be more important than others, depending.
