Well, I'm glad you gave this a chance and that it's working for you! As to your questions:
1. LOL. I recently added this question to my FAQ. Theoretically, it's just outside, but Dean obviously hasn't seen it and Sam doesn't talk about it, so its status remains a mystery … at least until later installments.
2. Dean's amulet was presumably taken from him with the rest of his clothes and personal affects, which he presumably doesn't have access to, but, like the Impala, it may reappear in later installments.
3. Ah, the changes. Well, there have been many changes, but I think in that context Sam was referring to Dean's ability to handle both of their very personal transitions, his own from a free hunter to an enslaved whore, and his brother's from a fellow hunter to a full-time supporter of the demonic cause. It's interesting that you brought up the environment, though, because it won't be explained/explored until much later, but I'll generally say, assume less that Earth has been made into Hell in a physical sense and more that Earth has been made into Hell in a socio-cultural/political sense. There is physical damage due to war and just demonic fun, but there is still certainly breathable air.
I'm glad you appreciate the POV, a lot of people are begging for a Sam POV, but the 'verse very much needs to be Dean and the readers trying to figure this all out, so ... yeah. I'm glad you're liking it! And thanks for the great feedback! :)
