Apologies for replying to your comments so late, but as for your question, it's not necessarily that this is when Sam's nightmares come true so much as you see him experiencing similar nightmares again, so it may be good, for continuity's sake, to remind yourself what that looked like and what it may have meant (or at least what Dean thought it meant).
